Italy coach Nick Mallett has made nine changes to the team to face South Africa in the second Test on Saturday.
Fabio Ongaro, Lorenzo Cittadini, Marco Bortolami, Carlo Del Fava, Paul Derbyshire and Manoa Vosawai come into the forward pack and Simone Picone, Michele Sepe and Gonzalo Canale are included in the backline.
Sergio Parisse continues as captain, a week after making a try-scoring return to action in the 29-13 loss in the first Test.
Former Springboks coach Mallett also makes a positional switch, moving Andrea Masi from outside to inside center, where he will win his 52nd cap.
Mirco Bergamasco is one of the six players to retain his place, and the wing will continue with place-kicking duties after scoring eight points with the boot in the first Test.
